AUGUST NURSES' SCHEDULE FOR PINK SLIPS AND PHYSICALS Thursday Aug 19th 8am***Doctor -Physicals **The doctor will only be at the high school for a short period. A student needing a physical need to be in the nurses' office 30 minutes before the doctor arrives with their signed Sports Questionnaire/Medical Evaluation form. (If you need a copy of this form you may find it under physicals or have their parent available to sign it before seeing the doctor.)
